GTN Sorbitrate CR 6.4 Tablet is a controlled-release (CR) formulation of Nitroglycerin (6.4mg), a nitrate vasodilator widely prescribed for the prevention and long-term management of angina pectoris (chest pain caused by reduced blood flow to the heart muscle).

The controlled-release form ensures a slow and sustained release of the drug, providing prolonged action and reducing the frequency of angina attacks compared to immediate-release formulations.

It works by relaxing blood vessels, reducing the strain on the heart, and improving oxygen delivery to heart muscles.


Uses:

  • Prevention and management of chronic stable angina.

  • Reduces the frequency and severity of chest pain episodes.

  • Improves exercise capacity in patients with ischemic heart disease.

  • May be part of therapy in patients with coronary artery disease (CAD).


Dosage & Administration:

  • Dosage must be as prescribed by the doctor.

  • Typically taken once or twice daily, depending on severity.

  • Should be swallowed whole with water; do not chew, crush, or break the tablet.

  • Not suitable for immediate relief of acute angina attacks (for emergencies, sublingual nitroglycerin tablets are used).


Mechanism of Action:

  • Nitroglycerin is converted into nitric oxide (NO) inside the body.

  • NO activates guanylate cyclase, increasing cyclic GMP (cGMP) in vascular smooth muscles.

  • This causes vasodilation (widening of blood vessels), reducing:

    • Preload (blood returning to the heart).

    • Afterload (resistance against which the heart pumps).

  • The net result: decreased oxygen demand and better oxygen supply to heart muscles, preventing angina.


Side Effects:

  • Common: Headache, dizziness, flushing, weakness, hypotension (low blood pressure).

  • Less common: Palpitations, nausea, blurred vision, fatigue.

  • Serious but rare: Severe hypotension, fainting, methemoglobinemia, tolerance (reduced effect with continuous use).


Precautions:

  • Not indicated for acute angina attack relief.

  • Must not be stopped suddenly; tapering is required under medical supervision.

  • Caution in patients with:

    • Low blood pressure

    • Severe anemia

    • History of glaucoma

    • Recent head injury or brain trauma

  • Avoid alcohol as it may intensify the blood pressure-lowering effect.

  • Contraindicated with phosphodiesterase-5 inhibitors (like sildenafil, tadalafil, vardenafil) due to risk of a dangerous drop in blood pressure.

  • Use with caution in pregnancy and breastfeeding—only under medical advice.


Storage:

  • Store in a cool, dry place, away from direct heat and moisture.

  • Keep the container tightly closed.

  • Keep away from children.
