Type 2 diabetes mellitus: For persistent high blood sugar in adults; reduces diabetes complications such as nerve, kidney, eye, heart, and vascular disease when taken as part of a comprehensive plan with diet and exercise.

Used when monotherapy or dual therapy is not enough for glycemic targets.

Glimepiride: Increases insulin secretion from the pancreas.

Metformin lowers blood glucose by:

• Reducing liver glucose production,

• Decreasing glucose absorption in the gut,

• Improving the body’s sensitivity to insulin.

Associated with stable or modest weight loss, low risk of hypoglycemia, and possible cardiovascular benefits.

Prolonged-release (SR): Usually started at 500 mg once daily with an evening meal; dose adjusted as prescribed, not to exceed the maximum recommended dose.

Swallow tablet whole with water—do not crush, chew, or split.

Should be taken at the same time each day for best results.

Dose should be adjusted by a healthcare provider, based on kidney function and glycemic control.

Common: Flatulence, abdominal discomfort, nausea, diarrhea (often transient), mild taste disturbance.

Rare but serious: Lactic acidosis (medical emergency: deep/rapid breathing, fatigue, persistent vomiting), vitamin B12 deficiency with long-term use.

Not for type 1 diabetes, diabetic ketoacidosis, severe kidney/liver disease, or in acute illness causing dehydration.

Regular monitoring of blood sugar, kidney function, and annual B12 level advised during therapy.

To reduce GI upset, always take with food.

Type 2 diabetes mellitus: Reduces postprandial (after meal) hyperglycemia and overall glucose fluctuations as part of a comprehensive diabetes plan.

Used alone or in combination with other antidiabetic agents. Often prescribed if monotherapy/dual therapy is not enough for glycemic targets.

Voglibose: Blocks intestinal enzymes (alpha-glucosidase) responsible for converting carbohydrates into glucose, thus slowing its absorption and preventing sharp rises in blood sugar after meals.

Helps lower overall HbA1c and reduces the risk of diabetes complications. Does not cause hypoglycemia by itself, though risk increases if combined with insulin or sulfonylureas.

Take one mouth-dissolving tablet just before the main meal, as prescribed by your physician.

Place on tongue; allow to dissolve completely. No water needed, but can be swallowed with water if preferred.

Do not skip doses. Continue consistent timing for best results. Do not stop abruptly—can cause elevated blood sugar and increased risk of complications.

Common: Flatulence (gas), stomach pain, loss of appetite, diarrhea, constipation, mild rash.

Does not slow gut motility, so risk of constipation/secondary infection is low compared to some antidiarrheals.

Dapagliflozin: Promotes urinary glucose excretion by inhibiting SGLT2 in the kidney, lowering blood sugar, also benefits cardiovascular and renal health.

Metformin: Reduces hepatic glucose output, increases insulin sensitivity, and slows gut glucose absorption, supporting long-term sugar control.

Type 2 diabetes mellitus: Lowers blood sugar in adults inadequately controlled by diet, exercise, and/or other oral agents, and helps reduce long-term diabetes complications.

Can be used in combination with other antidiabetic medicines (like metformin, sulfonylureas, or insulin) when needed.

Dapagliflozin: SGLT2 inhibitor—increases urinary glucose excretion, lowering both fasting and post-meal blood sugars, and offering additional benefits such as mild weight loss, blood pressure reduction, and heart/kidney protection.

Sitagliptin: DPP-4 inhibitor—raises insulin levels after meals, lowers glucagon (the hormone that raises blood sugar), and provides smoother postprandial glucose profiles with a very low risk of hypoglycemia.

The combination targets two blood glucose regulation pathways for more effective and sustained glycemic control than either agent alone.
